Support Coordination....
Support Coordinators assist people in the development of his or her personalized support plan in the areas of home, work and community to attain his or her desired dreams and goals. The development of the personalized plans are driven by individualized choice and natural support networks. Once the personalized plan is developed, the Support Coordinator works together with the person to endure the supports and services identified within the plan are carried out. Revisions to the personalized plan are done in accordance to the person’s desires. ADVANCE Support Coordinators are committed to obtaining and implementing best practices, state of the art principles and technologies that may enhance services. Support Coordination embraces the 21 Outcomes as prescribed by the Council on Quality and Leadership:

Residential Services..... back to top
.....provides the supports people need to live in and be an active member of their community. These supports are currently offered at seven group homes and two apartment buildings which provide staff supervision year-round, including holidays. We also offer supports to individuals who live either with their families or on their own in traditional community housing. The supports offered by staff in Residential Services cover a wide variety of daily living skills, such as:
- preparing healthy meals
- maintaining a clean and safe home
- taking medications
- shopping for groceries and personal items
- accessing transportation
- participating in recreational and community activities and events
- following Occupational and Physical Therapy recommendations
- assisting with personal cares
- managing finances
For people who desire to learn these skills and perform them more independently, staff provide training to help them do so.

.....are provided at the Training Center or through Career Advantage. Both programs are designed to provide vocational evaluation, training, and support based on individual needs. The Training Center provides training to a variety of people who are provided the opportunity to work and earn a paycheck. The work that is available is provided by area businesses such as Larson Manufacturing, Twin City Fan, 3M, and Daktronics, to name a few. Career Advantage provides vocational evaluation, job placement, and on-the-job support. Career Advantage serves a 6 county area for people who have cognitive and/or physical disabilities. These individuals are referred to Career Advantage via The Division of Rehabilitation Services, State Services for the Blind & Visually Impaired, School Districts, other professionals, and friends and family members. Staff are also available to provide businesses with information on disability related issues.

.....are offered as an alternative to employment services. Currently, day services are provided at the activity center and three residential locations. Individuals participate in many activities offered through day services such as: visiting the humane society, dancing, arts & crafts, shopping trips to Watertown, Sioux Falls, and other surrounding cities, the music lady, cooking & baking, visiting the zoo in Sioux Falls and Watertown, games, cards, socializing with friends, eating out, swimming and water aerobics, etc.

.....supports individuals to ensure their “best possible health.” Individuals are supported by medical services staff in assessing their health care needs and accessing quality health care services in the community. Medical services assists individuals in scheduling appointments with their primary care physician, dentist, eye doctor, nutritionist, speech therapist, occupational therapist, physical therapist, psychiatrist, counselor, and other health care professionals and specialists.

- Family Support 360 Program provides service coordination and financial assistance to families of children with developmental disabilities living at home and individuals with a developmental disability. The Family Support Coordinator assists families in developing an individualized plan based on their family needs, and can assist them in accessing a broad range of services and supports. The coordinator can provide ongoing support and assistance to the family as their needs change, and advocate for the family when needed. In addition, financial assistance with extraordinary expenses can be provided through Family Support.
- Eligibility: An individual must meet the following criteria to be eligible to receive services through Family Support:
- The individual is a resident of South Dakota
- The individual has a developmental disability or be identified as prolonged assistance (Birth-2 only)
- Can not be receiving services through another HCBS funding source or be living in a residential program setting through an agency(examples: Adjustment Training Centers, Assistive Daily Living Services (ADLS) Program through the Division of Rehabilitation Vocational Services).
